Projects Funded
by the Foundation for Healthy Communities
The Foundation for Healthy Communities has funded scores
of hospital-based community health programs since its
inception in 1994. Funded programs have included
community health needs assessments, primary and dental
health clinics and education programs, as well as projects
focusing specifically on childhood immunization, violence
prevention, elder care, end-of-life care and tobacco
use prevention.
Distribution of
Foundation Funding
Since the Foundation’s inception in 1994, $1,301,003
in general grant money has been awarded to Ohio hospitals.
During that time, the Foundation has received 511 grant
applications from hospitals throughout the state, and
has awarded 116 grants. Of those grants awarded, 40%
went to small and rural hospitals and 60% went to other
types of hospitals. Grant awards were evenly distributed
to all four hospital districts throughout Ohio with
a slightly higher percentage to the Central District.
