Grants Help Hospitals Partner for Better Health

2007 Grantees

COLUMBUS – Six Ohio hospitals are teaming up with local schools, social service agencies and others to promote better health for targeted local populations, thanks to $100,000 in grants from the Foundation for Healthy Communities

Marietta Memorial Hospital
 $20,520 for “Know Your Heart Risk: Assessment & Follow-Up,” a program to provide free EKG screenings/health assessments to the medically underserved populations in the hospital's service area. Marietta Family YMCA is a partner for this project.

Mercy Medical Center
, Canton
 $17,127 for “Project CARE (Creative Approach to Resident Empowerment),” A resident empowerment program that focuses on building a safe and healthy neighborhood at Skyline Terrace. Skyline Terrace is a subsidized housing community in Southeast Canton. Community Services and the Canton City School District are project partners.

Miami Valley Hospital, Dayton
 $13,544 for “Youth Creating Healthier Lifestyles II,” a program which will focus on minority young women ages 14-19 at risk for obesity and who are at high risk for developing health conditions related to obesity. Partners in this project from the hospital’s Mahogany’s Child program include the Combined Health District, Colonel White High School, the Dayton Black Nurse Association and Children’s Medical Center, Dayton.

 Mount Carmel Foundation, Columbus
 $17,675, for “Dementia Caregiver Wellness Program” through Mount Carmel Senior Services.  This program will offer wellness activities including exercise programs and respite care to caregivers of family members who suffer from dementia. Partnering are Heritage Day Health Centers and Alzheimer’s Association of Central Ohio.

WRH Health System, Wadsworth
 $19,850 for “Centering Pregnancy,” to empower women who are pregnant and at-risk  and their families or partners to take control of their bodies, pregnancies and families. Wadsworth City Schools will assist in this project.

Dunlap Memorial Hospital
, Orrville
 $11,284 for “Growing Healthy Habits, to provide educational and motivational opportunities related to fitness and nutrition for approx 500 students and their families in the Orrville City School district.

2006 Grantees

The Foundation for Healthy Communities is pleased to announce the four grants recommended for funding in the 2005-2006 grant period. These Healthy Hospitals grants received a total of $70,000 in funding.

East Ohio Regional Hospital, Martins Ferry
The program is designed around a NASCAR-race theme promoting sustainable participation in the wellness and health improvement among employees and family members with the primary focus on reduction in cardiovascular disease.

Because We Care

St, Elizabeth and St. Josephs Health Centers, Youngstown
The project will provide convenient, cost-effective employee access to the hospital's award-winning employee wellness program through Employee Health Kiosks strategically placed on the hospital campus.

Employee Wellness Program

St. John West Shore Hospital, Westlake
The Program emphasizes the impact stress plays in the hospital environment and ways for employees to cope with pressures and some of the results of stress such as weight gain, smoking and poor nutrition.

Memorial Hospital, Fremont

This wellness program will go to the next level by capitalizing on collaboration with local organizations to improve the overall health of the community.

2005 Grantees

The Foundation for Healthy Communities is pleased to announce the six grants that received funding in 2004-2005. These Healthy Hospitals grants received a total of $100,025 in funding.

Get-A-Life

Southern Ohio Medical Center, Portsmouth

The project sets goals for blood pressure, weight, tobacco usage and fitness and rewards participants with fitness center memberships or opportunities for a walking program.

O’Bleness Means Health

O’Bleness Memorial Hospital, Athens

A program to engage 25 percent of O’Bleness’ workforce to take initial assessments and set personal goals for exercise and nutrition. The program includes a smoking cessation and stress management component.

Shape Up, MMH!

Marietta Memorial Hospital
Identifies, educates and sustains motivation of employees with high risk for cardiovascular disease.

Strong and Healthy Employees

Euclid Hospital, Cleveland
A program with personalized clinical support as well as electronic communications to employees diagnosed with diabetes, high blood pressure, and/or high cholesterol.

Wellness At Work program
$17,720
Berger Health System, Circleville
A program to enroll 75 percent of Berger’s workplace into the Wellness At Work program. Employees will participate in a health risk appraisal that will be used to identify and track individual and corporate modifiable health risks to which education will be targeted.

Wellness Works
$20,000
Middletown Regional Hospital

Offers free health assessments to all participants, with referrals to wellness education, and, if needed, intervention. Participants will receive a monthly deduction on their health insurance premiums.

 

Healthy Hospital Grants Results

 On behalf of the Foundation, a professional evaluator collects data and compiles reports from the coordinators of the Healthy Hospital grants. The reports contain qualitative information on the programs as a whole as well as lifestyle and clinical measures of the participants in aggregate.
